Thank you for choosing us to take care of your family. Here is some information and forms to help get you ready for your first appointment.
For your first appointment, we do request you bring a copy of your photo ID, the insurance card for your child, and the following paperwork filled out, or arrive 15 minutes early to fill out all the necessary paperwork.
We participate with a wide range of insurance plans, but we do recommend contacting your insurance carrier to ensure we are an In-Network provider for your plan.
For newborns, we do give you until the child is 30 days old before we begin billing insurance. If your child is not added to the policy by day 30, we do have self pay options available.
For non-newborn new patients, please download and fill out the medical release form to obtain your child’s records from their previous doctor. Please also bring a copy of the child’s immunization records to the first appointment to ensure you are able to receive vaccines at your appointment.
